The log shows quite clearly the reader saying, over and over, that no 
medium (i.e., no card) is present.  So either the reader is broken or else 
it's not being used in the correct way, although I can't see anything 
wrong.

You said before that the same hardware worked okay with an earlier version
of the operating system.  Is it possible for you to get the same kind of
usb-storage debugging information from that earlier version?  The
difference between it and 2.6.11 ought to show where the problem occurs.
