You have to load ifsym with the option "misc":

You should delete it, because LyX uses inputenc automatically with the right option.

If you finish ERT-commands with a space, you can be shure to avoid LaTeX-errors. For a space behind the ERT-command in the output, use a protected space (C-Space), as in the attached file
