I got it working pretty reliably with these settings (MBP C2D):

  Section "Device"
  Identifier "ATI X1600"
         Driver "fglrx"
         #Option "VideoOverlay" "off"
         Option "UseInternalAGPGART" "yes"
         Option "TexturedVideo" "on"
         Option "DesktopSetup" "single"
         #Option "DesktopSetup" "horizontal"
         Option "Mode2" "1024x768"
         Option "EnableMonitor" "crt2,lvds"
         Option "PairModes"
"1440x900+1440x900,1440x900+1024x768,1024x768+1024x768"
         #Option "OverlayOnCRTC2" "1"
         #Option "OpenGLOverlay" "on"
         #Option "Centermode" "on"
         Option "XaaNoOffscreenPixmaps"
         Option "AccelMethod" "XAA"
         Option "AddARGBGLXVisuals" "True"
         Option "DisableGLXRootClipping" "True"
# Option "AllowGLXWithComposite" "true"
         BusID "PCI:1:0:0"

Note: to activate set "DesktopSetup" to "horizontal".

I use an external CRT running at 1024x768. With this setting
Xinerama support works quite well (No black borders or panning
of virtual screen size).
