Hi Jonathan,
If I recall correctly, LilyPondTool supported direct MIDI input. The developer
is no longer updating it. I used LilyPondTool with LilyPond versions thru 2.16.
You can also look at the oommand-line engine provided with LilyPond if you're
OK doing offline/file-based

I bet you can get more experienced voices to chime in if you post this question

I've used a command line utility calles "Rumor", describing itself in
the manpage as a "program for transcripting simple music played on
(MIDI) key‐board to Lilypond text notation." But I stopped using it as
it requires a lot of clean-up afterwards.
