Check out my TNTMonitoringPlugin project on Sourceforge. There is a plugin specifically to monitor Exchange. It handles both Exchange 2003 and Exchange 2007. It does monitor queue lengths as well as a few other factors, but the focus is not really so much on load. Queue length primarily is an indicator of network problems. A long send queue, in particular, tells me that the Internet connection is down, or something along those lines.
If you look at the source code for the plugin, you can also see the WMI query used.
