The documentation and the wiki show the syntax for \mar

         1       2
\mar [...,...] {...}
      OPTIONAL
1  REFERENCE
2  CONTENT

When I use the optional argument, then the [ is taken
as an argument for the content, which is correct placed
into the margin, but the rest "foo]{+}..." is taken as
default text. I suppose, that \mar has no optional
argument?

\starttext

\startitemize
\item baz
\mar[foo]{+} bar baz
\item barbaz
\stopitemize

\stoptext
