using latest beta from 2015.07.01 21:40, both options work for me:

    \language[nl]
    %~ \startexceptions[nl]
    %~ Ams-terd-am
    %~ \stopexceptions
    \hyphenation{Ams-terd-am}
    \starttext
    \hyphenatedword{Amsterdam}
    \stoptext

BTW, I have to change the hyphenation points, since the proposed
exception is the default hyphenation in Dutch.

Or how did you know that the exceptions weren’t working?
