This appears to be either a data validation problem or a documentation
problem.  When the request creation form asked for the Certificate Data
Name, I entered an X.509 name such as CN=Me,O=MyOrg,C=US.  When I
changed it to simply Me, it worked without error and I saw that the name
field was being used to populate the CN field.
