I'm using JTAG, not SWD.
And I found that problem triggered by EMI - error happens immediately when I 
touch by tweezers metal case of another device powered from the same power 
adapter. But shouldn't OpenOCD perform some actions to recover from such type 
of 
error? "Polling again" fails again and again because stlink_usb_xfer() got the 
same code 0x12 until OpenOCD not restarted.
