Question:

I've not been following the entire thread carefully, so don't know specifically what camera has been in play here, but... while the latest image is being written to disc, isn't there a little light somewhere, maybe up by the viewfinder, that blinks to inform you exactly that ~ that the system is busy writing to disc?

It's a software issue - some software is corrupting the table of
file/directory names. If it's a portable storage device, I would look
for a firmware upgrade or return the unit as faulty.

If you are on windows, try Scandisk. It might fix the table.
