Are you using PLA from sourceforge or from a distro? Yes, the wiki is 
out of date and does need to be updated.

The templates should be in the creation and/or modification 
subdirectory. I recommend you log a bug report with your distro packager 
if that is where you sourced PLA...

I've just committed some code to CVS to enable "disabling the default 
template". You can apply the changes - check the CVS for changes in:

config/config.php.example lib/EntryWriter1.php lib/config_default.php
