Looks like an issue with the artwork cache or you are using some image format iOS doesnt support. Or you have specifically set no image for that track. The image for the NowPlaying screen is an unscaled track image, the Playlist uses the album image first and then updates for the track image when (and if) that comes in later. Could you try to clear the artwork cache (or delete artwork.db on Linux setups)?
