I had a similar problem but I think the problem is the browser's cache. So
to avoid the kind of problem I had to change the URL of the img every time
like:
my_image.jpg?nocache=3432842
and when changing the image:
myimage.jpg?nocache=4382233
the browser will think that it's a different image and will reload it.
