I will also second this, as I am trying to find out information about the
journal Preaching : word and witness. Liturgical Publications.  But is it
published in Milwaukee or Berlin, Wisc. ?

I doubt Mac has overlooked the first level.  In practice that level was
seldom employed, or at least was augmented by details such as place.
Only for brief, preliminary entries was this truncation of data
tolerable.  Some BNB (UK Marc distribution) records of a previous period
omitted place, and it was a b***** pain in the neck to correct the
omission for catalogues that aimed to maintain traditional standards of
presentation.

While we may believe place of publication is not always significant, may
I point out that users of the catalogue following any of the style
guides are expected to provide place of publication in citations?
Omitting that data runs contrary to the principle of saving the user's time.
