OK, two of these bugs I understand and have written fixes for.  The others
I'm still trying to get a handle on.

I was finally able to reproduce something like it just once per
session, the first time any track plays.  Still working on what causes
it.

That bug I understand, and I'm pushing a fix for it.

Sorry, my oversight.  Percussion should have been handled specially;
it is now.

What surprises me is that it resembles something I saw when debugging,
that I thought I had fixed.  Instrument didn't used to tell
ControlBlock when it changed program, which caused something like you
describe.  But now it does.  That is, before the stuff I pushed today,
I already pushed a fix for that.  But you're still seeing it.  So I
don't know what to make of that.
