After installing guest image,
  Convert it from raw to qcow2 by convert command.

qemu-img convert -f raw disk0.raw -O qcow2 newdisk0.qcow2

Now you can mount newdisk  to empty VM with virt-manager.
Or you can edit DOMAIN.xml as follows..

/var/lib/libvirt/qemu/DOMAIN.xml:

It seems the directory '/home/squid' has 'home_root_t' type.
Change it to 'squid_cache_t'

 Reading later release of kernel did not mention this bug.
Upstream/CentOS community will not release this software fix. :-)

The solution is dkms.
  dkms-fglrx, dkms-ati
was provided by Mandrake/PClinuxOS/Ubuntu.
Only dkms-nvidia (for CentOS5) was provided at dag repository.

You can list configration file by rpm command.
rpm -qc dovecot

I was update successfully from 1.2 to 2.0. because of no local setting with 
dovecot.
(baseurl=http://dl.atrpms.net/el5-i386/atrpms/testing)

If you have local configration setting, these files has extension with
 .rpmsave .rpmback .rpmnew
You have to pick up settings from old configration to new configration.

Not supported GMA4500MHD(G45) with CentOS.
This hardware is equipped with notebook and blue-ray home pc.
CentOS/Upstream hardware supports until G40 VGA card.
I guess its ok to run dual monitor with CentOS on 4500MHD,
because G45 has a compatibility of G40 hardware.
Some trouble noticed at wiki,
If you want to enable all graphic features, run CentOS on vm with
other latest distribution(Fedora/Ubuntu/Win7 etc.).

Because your system has a driver to understand sysimage.
Install DVD only have major drivers.
If you have driver disk, Try this command with booting install DVD.
   boot: linux rescue dd

Should I try mounting my CentOS
 5.5 partitions manually at this point ?

If you don't have driver disk, boot with other live CD(Fedora, Ubuntu)
may include latest raid/disk driver can find your sysimage.

How about virtualization?(CentOS5.5Xen+4.8, Ubuntu10.04+Vmwareplayer+4.8)
Rebuilding kernel takes much times than making virtual environment.

 (I'm lucky if I can even do a yum update when the next version comes out!)
There is no update for latest hardware recognition
in CentOS4(in maintenance mode). This is upstream
maintenance policy.

He find that he removes 2-sata disks, It's booting fine.

the nash(finding volumes in boot sequences) has many
limits with boot device recognition.

resolution
  back to old kernel(5.5)
  rebuilding ramdisk(mkinitrd) with latest device driver.(if you have 
vendor/raid equipment)
  back to storage configurations.

changing grub/boot parameter with noquiet options show you debug messages.
these message can let you change your booting conditions.

I guess your your file system has a trouble with i-node(file number) resources.
CAUSES:
   Hard Disk trouble (bit error/raid trouble.)
   i-node trouble (overflow. etc.)
   Memory/CPU trouble(inode_lock)

Buy Fresh Hard disks rebuild them is convenience way.
Or memtest86 can finds DIMM trouble.(or CPU, mother board)
Or ext4 bug in 194.3.1 kernel, back to ext3!

You can check your .config file and CentOS config file.
(/boot/config-2.6.18-194.3.1.el5)
Many options must be =Y in the file.
and Re-building kernel is much taking your time.

Shortly, rebuild centos5.5 without LVM(root=/dev/hda) is a answer your question.

Anyway you can try,
   A. Add swapfile(swapon /dev/swap
 # (8G+SWAP)  (Unzipped5G.tgz+tar+gzip+file cache.+etc.),
   B. Buy more memory(go to radioshack).

The message seems  problem is on disk io.
This child_rip handler erases everything in shutting down sequences,
 but kernel object already erased.
Its cause from ..
1. some driver double freed a same object. (driver problem)
2. heavy new/delete events double frees a same object(environment problem)

To resolve your problem, keep your disk access stably.
I saw you use xen kernel to ask xen-expert.
HOST and GEST can't handle like memory problem on VM environment,
 Reduce I/O,
 Change your hypervisor.

you can try many version of kernel-xen on centos.(or Xenserver)
you can reduce your traffic on your environment.(or upgrade your system ;)
