I hit a similar issue to this at a site and it turned out some naughty changes had been made to web.config file in C:\Program Files\Common Files\Microsoft Shared\Web Server Extensions\14\CONFIG. I had to literally remove all safecontrol entries and add back until I narrowed it down.
