There is NO CRASH at all in my system, except for this annoying crash report 
EVERY time that I boot into Ubuntu 14.04
After a few months, I finally decided to look into it because it's getting to 
be pretty annoying, I thought it would have been fixed by now.
I will try the:

sudo chmod..

It seems like it's helped others, so I'm going to see if it works for me
and report back on whether it does or not.

With a btrfs install, I started getting the grub screen with the error
message:

'sparse file not allowed.
press any key to continue'

I tried the temporary fix by Colin on comment #10 which temporarily
fixed the problem.  After a few successfull boots without any error
messages, I then began to get a different error message:

'recordfail...
press any key to continue'

I then tried what's suggested in bug 669481

Unfortunately the problem was still there.
For now, everything is working ok.  What I've done is follow the advice on 
comment #10 from this bug, but I've also removed the #s from 
/etc/grub.d/00_header where it says:

   if [ -n \${have_grubenv} ]; then if [ -z \${boot_once} ];then   
save_env recordfail; fi; fi
}

Everything is working fine now.

Hello,
I've changed the status of this bug to Fix Released because whatever the 
problem was, it is no longer happening with the new version being used with 
Ubuntu 12.04.  I've now gone back to using Rhythmbox instead of Banshee because 
the problem I was having has gone away.  Whatever it was, it has been fixed and 
everything's working fine now.  Thank you.

I have changed the status of this bug because I am now using the computer which 
originally was giving me problems and it is now working fine.  I upgraded to 
Xubuntu 12.04 and everything works perfectly out of the box.  I upgraded with a 
Live CD from Ubuntu 10.10 to Xubuntu 12.04 and had no problems whatsoever.  The 
upgrade allowed me to keep my old files and settings  and was rather quick and 
painless. 
  To anyone who might have had problems with the 11.04 release, I strongly 
recommend upgrading to 12.04 and you should have a good, solid working system 
in no time.  I initially tried Ubuntu 12.04 (Unity) with a Live CD and although 
the screen wasn't completely blank as before(11.04), it was still pretty hard 
to see anything.  However, with Ubuntu 12.04 I could at least switch to a 
terminal session, which I couldn't do before--that's why I was never able to 
submit any sort of bug report, because with 11.04 NOTHING was working, not even 
a terminal to get apport to gather the bug report.  
  So after noticing some improvement with Ubuntu 12.04 I figured I'd try 
Xubuntu 12.04 which uses the Xfce Desktop environment and is quite a bit more 
compatible with older hardware than Unity is.  After noticing that my old PC 
was responding well to the Xubuntu 12.04 Live CD , I decided to perform the 
upgrade.  The upgrade worked well and as I stated before, it was also very 
quick.  I am now writing this with the PC that originally had problems with 
11.04 and it is obviously working very well with 12.04.
  I don't claim to know what the cause was, but my guess is that it might have 
had to do with the (old) Linux Kernel's problems with Power Management from a 
year ago.  Whatever the cause might have been, I can very honestly say that 
everything has been fixed and it is all working fine.  So I hope this helps 
anyone.  Try upgrading to 12.04 and everything should work out of the box (try 
a Live CD first, just to make sure).

I am affected by this bug.  I first noticed during Oneiric Ocelot, but it 
happened again today and I am using Ubuntu 12.04.
I've had to switch to a CLI terminal by using Ctrl+Alt+F1 so I could reboot 
because that was the only way I figured I could do it.

Christopher M. Penalver, I don't know if the bug I reported is still valid as I 
no longer use the computer which had the problem.  I upgraded to a newer 
computer, which is actually what I'm using right now with the Oneric Ocelot 
release and I have no problems whatsoever.  I will try my best to find answers 
to your questions but it may take me a while since I've given away that old 
computer to my mom and she uses Windows XP on it which is installed as a dual 
boot option.
Now, seeing that other people were also affected by this bug I don't feel right 
changing the status to fixed and /or adding/removing tags.  Give me a couple of 
weeks so that I can test the problem computer and hopefully give you some 
results then.

Gabriel, I will try what you suggested.  By the way, I have been using Ubuntu 
Studio 11.10 and ever since this problem started, I switched to Banshee in 
order to listen to music.  In prior Ubuntu Studio releases (11.04 and older) I 
didn't have this problem.
  I already tried the update option and that didn't work, so now I'll try the 
re-installation idea.  I'll post something here as soon as I find out if the 
Re-Installation works  or not.

where is the 'smooth effect' that you tell me to de-activate?  Do you mean 
under Preferences-Playback-Crossfade?  That's the only place that looks like 
what you are calling 'smooth effect' and I disabled it but still the problem is 
there.

I had to declare this bug as New again because originally it was declared 
Solved by Gabriel Genois who is the person responsible for reporting it in 
2009, but it was in fact never solved.  The symptoms described are the same 
ones that are happening when I use Rhythmbox.  After I begin using it, I can 
listen to the first couple of songs, but then everything in Rhythmbox freezes 
and when I press Pause and then Play again, Rhtymbox crashes.  Having 
closed by itself, I have no other option than to re-start it but that's kind of 
a hassle to do everytime I want to listen to music through Rhythmbox.  I have a 
crash report that I gathered from the last crash and I will post it here 
immediately after posting this comment.

Rhythm Box will play a song or two, or three, then it will pause and when I try 
to get it to play again, it will crash.  Prior to today, it never did that.  
There were about 75 updates available today on the Ubuntu Update Manager and I 
updated almost all, except for two which I know I definitely do not need (I am 
not blind, nor do I have any hardware that a blind person would use so I 
believe I don't need to update any support for braille, which are the two 
updates that I declined).  The update didn't seem to have anything that 
directly referenced Rhythm Box.  Prior to today, Rhythm Box was working fine 
and I never had any problems with it.  I realize that as of lately, Banshee is 
the default music application, but ever since I started using Ubuntu, I got 
used to Rhythm Box, which is why I still use it.  I tried Banshee a couple of 
times, but Rhythm Box felt easier to me.  Yesterday, I had no problems with 
Rhythm Box but today I do.  I could be wrong but I am almost certain that the 
problem is directly linked to one of the updates from today, but being that 
there were 75 updates and none of them directly referenced Rhythm Box, it is 
too hard for me to pinpoint the possible perpetrator.
I am using Ubuntu Studio 11.10 Desktop 64 bit

question was reported as solved, I didn't see an explanation as to how to solve

Well, I am definitely glad I decided to report this bug as it seems very 
evident that I was not the only one affected by it.  I am very grateful for all 
 your support in trying to help me fix my problem.
  The Ubuntu community is definitely one of the main reasons why I decided to 
migrate to Linux from Windows and over the past month I have really come to 
love being part of such community.  I only hope that as time goes on I can be 
of great help to others who might be asking questions to answers I might have.  
  Unfortunately, for now, as much as I hate to say it, I have thrown in the 
towel and given up on finding a fix for my bug.  I upgraded another computer 
(my friend's eMachines ET1161 desktop) and again I ran into some unfortunate 
issues with the Nvidia graphics card.  The problem on her computer was not as 
bad as it was on mine, but it was still enough to leave me with a bad taste in 
my mouth.  On her computer everything really seemed to be working perfectly 
fine (unlike mine, which seemed to be completely broken).  However, I noticed 
that her 3D graphics effects acceleration was not up to par when compared to 
how it used to be when she was running the Ubuntu 10.10 Maverick version.  As a 
matter of fact, even though the 'most recent Nvidia driver' had been 
successfully installed, for some reason I couldn't get it to actually work.  I 
tried a couple of different tricks and every time I would get the same message 
saying that the driver was installed but not being used at the time (kind of 
weird, right?).  Since, my friend is a great artist (paintings, drawings, 
photography, video, graphics), we decided that it would be in her best interest 
to have a working graphics driver so we reverted back to Ubuntu Studio 10.04 
LTS.  She is now happy  and completely worry free.
  As for me, I have also decided to go back to using the really STABLE Ubuntu 
10.04 LTS.  When I was using Maverick (10.10) everything was fine but since it 
is not a long term release I figured it would be best to stick to Lucid Lynx 
since that release is a Long Term Release and Maverick isn't.  I unfortunately 
gave up on trying to work with the new Natty.  Hopefully they'll get it right 
by the time that Oneiric Ocelot comes out in October and get this thing fixed.  
  As the saying goes, if it ain't broke don't fix it.  If the Nvidia drivers 
had been working just fine, I don't quite understand why things had to change 
with this latest Ubuntu release.

I would like to thank everyone for contributing to my bug report.  As I 
originally stated, apparently I am not the only one to be suffering from the 
Natty update.  Everything was fine with my system until the recent update and 
now I am glad I never gave up on Windows by keeping it in my dual-boot system 
as it is now through Windows XP that I find myself writing this.  Unless I find 
something that works soon I will most likely be reverting back not to Ubuntu 
10.10 but rather to the 10.04 LTS so that I don't have to deal with any updates 
that completely kill my system (which is exactly what happened) for at least 
another year.  
Canonical really needs to re-evaluate their strategy to use this new Unity 
desktop because it seems to really be hurting the momentum that Ubuntu had 
gained while using GNOME as the default desktop environment. Perhaps Unity is 
the right choice for netbooks but definitely not for desktops, at least not 
yet.  I, for one, would never have decided to switch over to Linux if my first 
experience with Linux had given me half as many headaches as the current 
situation that Natty Narwhal is making us endure. Thanks again to everyone and 
don't give up.

I had been using Ubuntu 10.10 ever since its release without any
problems but when I updated to 11.04 even though apparently everything
went well, I always get a black screen and cannot boot into Ubuntu
anymore.  I have looked for support in launchpad as well as the ubuntu
community forums and I have come to find out that this is actually a
pretty common problem.  If this isn't a bug then I don't know what is.
I am debating re-installing the good old Ubuntu Maverick Meerkat and
saying good-bye to this Natty Narwhal until they figure out this
problem.  I thought that a stable release was supposed to have all of
these issues sorted out and fixed during the alpha and beta stages.  I
guess I was wrong.  If anyone knows how to fix this, please let me know.
I have tried all of the suggestions regarding the Nvidia cards (which I
do have) and nothing seems to work.
