I have a similar problem (if not the same). I figured out that keyboard shortcuts operate on Master Volume instead gnome-volume-applet let you choose the channel to track. In my case, the channel Master doesn't affect the output volume at all. The real master volume is instead Headphone. This is a common problem due to non perfect sound card detection at kernel-level, and explains the channel chooser option in gnome-volume-applet. I have discovered a workaround. Deleting the .pulse folder works only temporarily. Once the user logs out the issue returns upon log in. I am having the same issue but on Jaunty, 2.6.28-14-generic, pulseaudio 1:0.9.14-0ubuntu20.2 gnome-settings-daemon 2.26.1-0ubuntu2. XF86AudioRaise/LowerVolume are recognized but are not having any audible effect, nor affecting any mixer control. Pause/Play and Prev and Next work fine though. gnome-settings-daemon issue? I'm pretty sure you wanted to report this against the gnome-media package, which contains gnome-volume-control, rather than against gnome- volume-manager, which is for things like CD autoplay and automounting.
