I can't answer why it's happening, but I've thought that it would be a
good idea to establish some kind of semi-comprehensive FAQ on basic
problems such as this one, and then to point users to this faq (presumably
on xfree86.org) as a preliminary measure in their crash messages. The
crash message should tell them that if it can't be resolved quickly that
way, then they should mail this list. That would probably kill all
these 'fixed font' errors. As far as I know, however, the FAQ
that was on xfree86.org was taken down, and there is no official
substitute as of yet.
