In function axis2_op_register_op_ctx() failure is logged and returned in a
working path.

1. Implement Policy Intersection support as described in Web services
policy spec 1.2 section 4.4

Yes this looks good. However you might also have to think about
specifying the namespaces that are needed in xpath processing too.
Probably use a xpath context and add a namespaces map to it?
