If I were doing that in orgmode, I think templates would get useful fast
to standardize notes formats. That's possible with standard tags and
associated fields. Beginnings of records would have tags identifying the
record type ((room)1) could be the tag for room 1 and ((player)george)

This probably isn't a quick fix, but I use Emacspeak and orgmode,
which is a pretty comprehensive notetaking system. Emacs has a
notoriously steep learning curve, however. Emacspeak is best on
Mac OS or Linux.
Plain text is the way to go, I think.
