Does the user nagios have rights to nrpe binary and config file?
Check nrpe.cfg for nrpe_user=nagios and nrpe_group=nagios.
To activate logging:
As default, nrpe log to syslog BUT you have to add daemon.debug to
/etc/rsyslog.conf :
still unsure if I missed something completely because the problem
is just so serious...

1- Configurar tu SSH de tu VPS por un puerto diferente al 22 (solo
recomendación)
2- Luego en tu Servidor Local deberás hacer una conexión y levantar un
puerto local en el cual dirigirás todo el tráfico.
Pudieras buscar en google. SSH Tunel y varios link te saldrán.

Permites relay en tu server postfix yo empezaría por deshabilitarlo, y lo
otro seria limitar el numero de correos enviados por minutos para detener
el spam eso lo haces con,
