[ACS41][BLOCKER] CLOUDSTACK-1745: template_s3_ref Table Definition Missing

All,

The template_s3_ref table definition is no longer in the create-schema.sql 
script.  Without this table, the S3-backed secondary storage feature will not 
function.  I have created CLOUDSTACK-1745 [1] to track resolution of this 
defect.
