Hello ,
I'm having the same issue. (I thought I was alone ;-)
I use Firefox (last version) in Ubuntu Hardy + Exaile + VLC
My sound card is integrated in the motherboard (NVIDIA nForce2 chipset + AMD
Athlon XP 2600+)
Test case Number 1 :
Using Firefox, I go on a WebSite that will catch my sound.
Mistake :
Then stopping Firefox
VLC (video): Sound OK +video OK
Totem (video): Sound OK +video OK
MPlayer Movie Player (video): Sound OK +video OK
Audacious (music): plays music OK
Exaile (music): plays music OK

Rhythmbox cannot play audio after watching Flash video in Firefox
I have the same problem on two machines running ubuntu 8.04 with
different sound cards, etc.
It looks like that if an application uses audio no other can use it
until that application is closed. I've tryied it with rhythmbox + flash
, rhythmbox + skype , skype + flash , etc
