I must say that I'm a bit surprised that you didn't take the effort to reproduce this problem yourself. Especially since fillable PDF forms are used for important things such as filing taxes. Also, I'd raise the priority because of this reason. In any case, I have attached a fillable PDF that

Jorrit, I cannot fill forms with adobe reader in linux (missing plugin?). However, after opening the pdf with evince, selecting option 1, saving, closing and re-opening it, the choice remains 1, so I cannot reproduce this. Do you want to attach the pdf with option 1 saved by Adobe Reader? --
