I must say that I'm a bit surprised that you didn't take the effort to
reproduce this problem yourself. Especially since fillable PDF forms
are used for important things such as filing taxes. Also, I'd raise
the priority because of this reason.
In any case, I have attached a fillable PDF that
Jorrit, over here the file you attached renders fine, see attached evince
screenshot (from jaunty).
Have you tried with the latest Ubuntu version?

I'm running Ubuntu 8.10 (intrepid), kernel 2.6.27.11-generic, gnome
2.24.1 with all automatically pushed updates installed.

Jorrit, I cannot fill forms with adobe reader in linux (missing
plugin?). However, after opening the pdf with evince, selecting option
1, saving, closing and re-opening it, the choice remains 1, so I cannot
reproduce this.
Do you want to attach the pdf with option 1 saved by Adobe Reader?

You can easily reproduce this with 1040NR tax forms for 2008.
Given the kind of problem and the place where it shows up (filing tax
returns) I'd certainly jump the importance to high.
