I have the same bug on Hardy. Using single monitor. NVidia driver version 177.
Clicking the Leave Full Screen button does nothing except make the button
react as normal.
In the view menu, the full screen option is greyed out.
This happens as soon as you either open Totem from Applications/Sound
Patch resolves the bug.

I've got a Sony Vaio SZ650N/C with the same problem. I am running the
AMD64 version of Hardy. I've done an acpi_listen and closed and reopend
the lid - there are absolutely no events listed there, nor in the
acpid.log. I've chatted with the HAL folks, thinking that the problem
might be there,

that's not a totem bugs, those effects are gstreamer one and require
ressources to be rendered, there is already some bugs about the need to
optimize the cpu usage so closing this one

thank you for your bug report, that's not really a bug though, could you
open the issue on bugzilla.gnome.org to see what the people writting the
software think about the issue? there is no realiable way to play over a
corrupted file so going to next one is just one could just be a choice
