That fixed it! Here are the details for future reference:

I went to C:/DX4W904  note: when I did the update to 9.0.5.1221 it used the 
DX4W904 folder. It did not update the folder to DX4W905. I see no problem with 
this

I opened the SAVE folder

I found the CONFIG.DAT file and renamed it to CONFIGXX.DAT

I opened/started DX4WIN

Only the main QSO window opened (as expected)
When I opened the WINDOW drop down menu I observed none of the options had a 
check mark. As I placed check marks in front of COUNTRY, IOTA, SAME CALL, WORLD 
MAP (one at a time) the windows opened and I was able to move them to where I 
wanted to place them.

I closed DX4WIN and went back to C:/DX4W904 /SAVE and observed there is now a 
new CONFIG.DAT file. I deleted the CONFIGXX.DAT that I had renamed.

I reopened/closed DX4WIN several times and all is FB.

HI Alan,

There used to be a preference "Leave all Windows in original position"
but I can't find it in 9.05.

Try this:

1.  Exit Dx4win
2.  rename SAVE\CONFIG.DAT to some other name 3.  Start DX4WIN

All your preferences will be reset, but can you now turn on the missing windows?

When creating a membership list, I did not filter an empty line which created

an entry for an empty callsign. When starting a filter, the callsign field is 
cleared but the membership field was initialized using the empty callsign (the 
@ in both cases)

 

The search failed unless you searched for a callsign in that @ list!

 

So the quick solution is to recreate your list but watch out for empty lines.

 

My new code guards against this happening in the future.
