ffmpeg -i input.avi -f mp4 -vcodec copy -an output.mp4
This causes the output to have its frames play back in an improper order, most
likely from how B-frames are packed within AVIs in comparison to MP4s.

Your patch is buggy, it sets has_b_frames to a variable that may or may not be
initialzed at that point, you provide no sample and no hint on how the pts are
invalid.
