skip_first_b_frames.m2v in incoming
mpeg2dec does decode correctly the 2 first B frames.

the frame rate.
r_frame_rate is just a guess and see documentation in avformat.h for more

time_code might not be reliable, I have files having gop headers time
code set as 00:00:00:00, even recommended by EBU when time code
reference is located elsewhere.
So yes, parsing the whole file is the best and only right way to compute
frame number.

libx264 is not generating incorrect extradata, it outputs NAL units containing
PPS, SPS, SEI.
Look at how mov muxer generates avc1 atom.
When I added H.264 muxing feature it was decided that it was muxer job to
correctly format extradata
