I have always created a separate file with that master page and then added that
file to my book. I do 8.5 x 11 and sometimes 8.5 x 14 which would be a foldout.
Not sure if there is another way.

You can, though, have Master pages with different text frames and crop
marks, if your production unit can handle that: Equally sized PDF pages
throughout the PDF (say, 12 x 9.5) but where some of them have a text
frame and crop marks corresponding to 11 x 8.5 and some to 5.5 x 8.5.

Because build expressions are barely documented in Frame 9 I've got a few
more questions.
It seems that every build expression ever used in the books I'm updating is
available in the list. Is there a way to delete unneeded build expressions
from the list? Can one name them somehow?

1. Save the FrameMaker file as .mif
