Ken: for those we usually just guess. I mean if you're about to emerge foo then you run `emerge -pv foo` first, which shows which USE flags are selected and which packages will be drawn in. If you see more packages being installed than you might expect then edit your USE flags in make.conf & `emerge -pv foo` again to see what the effect is. But mostly, if you don't know what's what, you can leave the USE flags empty & rely on the default ones.

For CFLAGS see http://gentoo-wiki.com/Safe_Cflags#Athlon_.28AMD.29 You can find out processor model & stepping with `cat /proc/cpu`
