I had this problem and found it was due to the version of ffmpeg.exe
being used.
I downgraded to this one:

I can't quite remember how I was able to tell I file was bad, I think I
was using one of ffmpeg's own tools ffprobe? to view the file data.
Unfortunately I've deleted the bad files now so don't have one to look
at. If you compare the bad files against good ones, you should be able
to spot

Okay, I built ffmpeg version 0.8.3 as suggested :) The only gotcha is
for some bizarre reason the ubuntu forum instructions on how to compile
it bizarrely install everything to /usr/local/bin, so it is not found. I
simply added -prefix=/usr to the configure options and then it all works.

It works OK for me. And other R4 is working OK also. I use the command
prompt in linux and have found no problems. This does not help, sadly,
except to confirm that there is not a generic BBC R4/Ex problem.
