I have encountered this as well. There are no errors. And after looking
through the user table in the database, the avatar file name column is not
being set.

Could it be the permissions on your public/system folder? The account
your application server is running as should have write permissions on
that directory to create {avatars,group_avatars} in there.

If there is permission, it should displaed in logs
