Mahler 4 starts with sleigh bells (or whatever they're really called).
The Rachmaninoff 2nd piano concerto starts, like the Beethoven 4th (my
favorite piece of music), with the solo piano. Prelude to Act I of
Tristan starts with a cello solo.
