hi
i have a pre-made pdf file with a field called 'name'. i like the pdf file
as is
but for the user, i want them to fill out a web form that has a field called
'name'
the web form and pdf file are in the same folder on my server
when the user clicks submit i want the field data they typed in to the
'name' field to transfer into the 'name' field on the pdf file, and then of
course have the pdf file delivered to the user so they can print it out
i would also like to export the data to a MySQL database, or have it
compiled as a fdf file and either saved in the same folder or emailed to me
is this possible? what is the best way to do this? i have spent a zillion
hours trying to get an answer. any help would be great
