It might also be useful to have output of
sudo sfdisk /dev/sdb -l
(assuming that sdb is your usb device node that your are formatting)

Which partition table did you use GPT or MBR? Might it be that windows only
recognizes MBR devices?
That said, I don't have Windows, so I wouldn't be able to test it.
