Has happened several times to me in the last week including today.
I have multiple smtp / pop / imap accounts
no master password on kwallet

If kwallet save 1 password, the problem disappears.
My temporary solution was 2 accounts, only save one password.

In my case it looks like that:
- no master password
- multiple SMTP accounts
- only alphanumeric passwords on all accounts

seems to remember it.
