In my opinion it's up to the OS or desktop environment how to handle main menu,

The Breeze icons are very controversially discussed. IMHO it should be the
default for KDE only.

Confirmed all of the above on Win 8, 8.1, x64.
The most surprising is that it worked well in all OO/LO previours versions we
used in the company, that is OO 3.2, LO 4.2.
