Problems in vertical spacing on header

Hi all, I would like to know what is in the subject. The subtitle is very long and I would like to put an A capo somewhere. For the names of instruments, the original score have for the voices "Soprani o Tenori" and "Contralti o Bassi". They are written in a long line. I would like to obtain
