any outgoing email with multiple recipients so please bear with me if it isn't
even a MailMan issue...

Seem to be getting this a lot on f...@here - spam comes from ffml-owner,
but that address is not subscribed, so it bounces, and bombards six people
with rejected spam. Is there a more elegant way to deal with this than add
subscriber to a 'summarily discard all' rule?

attached files in the received message.
Last time I tested it: I attached to the outgoing message 2 files. I
received a message with the 2 attached

The moderator of one of my lists has received a request for approval but when
he clicks on the link, there is no message to approve. Any help?

That looks like Exim, in which case I believe the explanation (complex) is

Either there is an old or test or backup installation that is sending
the moderator notice
