I removed the rpm and recompiled mutt and changed these two options: --enable-flock --disable-fcntl It seems fcntl doesn't work to great with NFS. Also make sure you have this line: set dotlock_program = "/usr/local/bin/mutt_dotlock" Or wherever you have mutt_dotlock installed. Works fine for me now.
