setting the align-option of \setlayerframed causes a small (but visible) right
shift.

\setupbackgrounds[page][background=page]

\setlayerframed [page]
[x=\backspace,y=\topspace]
[width=broad,height=\headerheight,align=right] {Not OK}
% [width=broad,height=\headerheight] {OK}
\page[empty]
\stoptext
