I used "Clone this bug" from another bug to create this one. I think maybe I
should stop using that feature, it doesn't do what I expect. Please clean up
anything that shouldn't be there.

For check-marks problem: open document, show forms, check box 1B "Normal
Distribution–Roth IRAs", then check box subordinate to that one, "This is a
Qualified Distribution". Save document. Open document again,
