are you in archive log mode? If so this is possibly
a sign that your archiver cant keep up with the amount of redo to archive.

Furthermore, did you notice existence of beautiful files
named something like *SMON*.TRC or *PMON*.TRC

archivelog mode ? Is it possible that the database is in archivelog more and the
archiver is not running ?

Unfortunately, there are numerous security fixes that have been made available that are not in a full patchset

the archive destination is full then for sure the database will stop but it
will not crash.

probably a bug, you should probably go to 8.1.7.4.
