[phpldapadmin-users] Multi-value attributes in Create template

Hello, I'm trying to make a create template, and I want to have 2 text fields for the Street attribute. I've tried putting in 2 of the tags, both with id=street, and that doesn't work. Can anyone provide some ideas? Also, where do I specify the types of entry for each attribute? Text area, text
