I seem to have quite the same problem, except that it is happening with
Squid-2.7 and -3.2 (I tested the newest version yesterday) on the OpenBSD
operating system.

Without digging deep into the source code, what is the function to
look up client's IP address in Squid? We are having some issues with
ISP doing rotating IP address. Our php is able to detect a static IP
but squid is getting multiple different IP address.
Help is appreciated,
