I'm tiring to run Squid 2.5.STABLE5 with ldap_group authentication.
I configured the squid.conf file like:

configure options: --enable-auth=ntlm,basic

and tested it without authentication - works fine.
I tested the Test the Samba-3.x helper - works fine
I added the relevant auth_param's and adjusted the acls in squid.conf - no go :(
I use IE6.0

If so, just back up your squid.conf file, or any additional config files.

Then run the exact compile string you used for the previous version.

Btw, I would also advice to check whether this mem. and crash
problem persist if you leave 'cache_mem' to it's default value
in squid.conf as a test.

If you set client_netmask to 255.255.255.255 it will block ip addresses
from being logged. That is the whole point in client_netmask.
