I ran into this first on a 16.04.5 install I have. I just retried it on
a new laptop with 18.04.1 with the same result.
When booting, I chose the advanced form, the rescue variant, and tried
to use its fsck option. This failed almost immediately. I'm attaching

For about 5 days now, I've been experiencing random freezes of the X
session.
When this happens, I can still move the mouse pointer and it does change

A the moment I can see following solutions:

Make the lvchange_activate function in
/usr/share/initramfs-tools/scripts/local-top/lvm2 a while loop which executes
the "lvchange" command repeatedly until the root LV appears
(2)
Utilize udevd to create the LV devices while wait-for-root is
I owned a Surface Book and now own a Surface Book 2. And both were/are
afffected by this. Still with kernel 4.20-rc5 the disconnect problem
persists after long heavy-usage. Does Ganapathi have some updates yet?

confusion in the LDDEBUG examples cited comes in). Comment #4 has a
good breakdown of why this is happening, but this is very much not a
glibc/ld.so bug.
