I *think* this was triggered due to using a shared /boot partition
between different distro installations (Ubuntu 32 bit, Ubuntu 64 bit,
Centos, etc).

Bugs, which is the
Solved by copying /lib/modules/* to /target/lib/modules under the
installer. The kernel modules were not installed when initramfs-tools
was trying to build the initrd, so it failed.
Stilll don't know why kernel modules were not installed in /target at
this point.
